Acupuncture in the management of asthma

In cooperation with the patient’s treating physician, acupuncture for the management of asthma can be used alongside other treatments, whether conventional or alternative.

Allergic asthma: clinical presentation

The symptoms of allergic asthma vary from person to person in both frequency and severity.

They generally include:

  • Cough (initially dry and irritating), which often precedes the actual asthma attack, especially at night and early in the morning.
  • A feeling of tightness in the chest.
  • Dyspnoea (difficulty breathing).
  • Wheezing (a whistling sound), especially during exhalation, due to restricted airflow.
  • Sleep disturbances.
  • Difficulty during physical exercise.

Research questions and limits of the evidence

  • Changes in symptoms require clinical assessment
  • A laboratory marker does not establish overall chemical balance
  • Universal strengthening or balancing of the entire immune system has not been established
  • Potential risks require explanation and assessment
  • Prevention of attacks cannot be presented as a guaranteed outcome.

Auricular acupuncture is described as a microsystem involving points on the ear. Its dissemination and teaching do not in themselves establish effectiveness for asthma or a blanket endorsement by the World Health Organization. Each proposed use needs evidence for the particular goal, discussion of risks and coordination with the treating clinician. Ongoing respiratory care continues, and recognition of terminology or training should not be interpreted as a guarantee of symptom control.

Asthma and complementary acupuncture

This asthma overview remains connected to ongoing respiratory care. Perceived comfort and objective measurements are not identical.

For a description of the method, see Medical Acupuncture Methods. Related reading: Respiratory System Conditions.

Start with the respiratory diagnosis

Wheezing, coughing and chest tightness need assessment as respiratory symptoms. They should not automatically be attributed to allergy, and they do not establish an asthma diagnosis on their own. The medical history, examination and appropriate breathing tests help the treating clinician understand the problem. Useful details include when symptoms appear, whether exercise or a particular environment brings them on, and whether sleep is disturbed. Any discussion of complementary care starts from this clinical picture rather than replacing it.

A brief record can include night waking, activities interrupted by symptoms and use of medication as prescribed. Its purpose is to improve communication, not to encourage independent changes in doses. Instructions for inhalers and the personal asthma action plan remain separate from an acupuncture appointment. Checking inhaler technique and raising questions at a review can be practical parts of everyday management. A feeling of relaxation after an appointment does not establish that airway inflammation or attack risk has changed.

Understand what the studies actually tested

The publications listed in an overview may investigate quite different procedures. Needle acupuncture, preparations applied to acupuncture points and other techniques are not interchangeable treatments. Readers should identify which intervention was studied, whether participants were adults or children, what usual medication continued, and how long observation lasted. A change in a laboratory marker does not by itself demonstrate fewer severe asthma attacks. Similarly, a result from children cannot simply be assumed to describe an adult with a different clinical history.

The joint BTS, NICE and SIGN patient information states that evidence is insufficient to suggest acupuncture can control asthma symptoms. This limitation matters when reading an older positive report. Comfort, quality of life, lung function and frequency of exacerbations are separate outcomes. Improvement in one should not be presented as proof of improvement in all the others. A small study can suggest a question worth investigating without establishing a reliable treatment recommendation or a personal prediction of benefit.

Coordinate safety with ongoing treatment

Before considering an additional procedure, tell the relevant professionals about inhalers, other medicines, previous severe attacks and approaches already tried. Acupuncture is not treatment for an acute asthma attack. Markedly worsening breathing or failure of prescribed reliever treatment to help as expected in the personal action plan requires urgent medical attention. An appointment for complementary care should never become a reason to wait for assessment of such deterioration. The treating clinician remains responsible for decisions about respiratory medication.

A natural or non-drug description does not mean a procedure has no risks. Ask how the technique is performed, which hygiene measures are used and what adverse events are possible. If a complementary session is chosen after discussion, agree what will be monitored and when it will be reviewed. Planned respiratory follow-up continues regardless of the immediate impression after a session. There is no universal course of acupuncture that can be promised to eliminate asthma or remove the need for prescribed care.

Clarify the service and its cost

Financial information should explain the initial consultation, what a session includes and whether a separate review is proposed. Ask how the stated goal relates to the available evidence and what would justify continuing. A price is information about a service, not evidence that it prevents attacks. Clear discussion of uncertainty helps patients weigh time, expense and preferences alongside established respiratory care, with realistic expectations and an agreed opportunity to reconsider the plan.

Sources for further information

Respiratory symptoms and assessment

Does a symptom change mean I can alter asthma medication?

No. Medication and the management plan are agreed with the treating clinician. A general acupuncture article does not instruct readers to change them.

Can an article about complementary care change respiratory medication?

No. Diagnosis and the treating clinician’s instructions remain the reference point. Any additional approach should be discussed with them, particularly when asthma or another respiratory condition is present.
